Medical Release

I hereby authorize the adult staff of Henfield Academy to act on my behalf according to their best judgment in any emergency situation requiring medical attention, and I hereby waive and release the Academy, its staff and affiliates from any and all liability for any injuries or illnesses incurred while training. I have no knowledge of any physical impairment that would prevent my child from participating.

Likeness Waiver

By agreeing to this, I allow any photos or video taken by Henfield Academy to be used in any marketing the Academy may undertake.

Liability Waiver

I/We, the undersigned, our heirs, executors and administrators, waive, release, and forever discharge Henfield Academy and its staff from all rights and claims for damages, injury, or loss to person or property which may be sustained during participation in training activities. I acknowledge the risks and hazards inherent in participating in this program, and that no accident or injury insurance has been provided for participants.

Academic Records Release

I authorize Henfield Academy to request, receive, and review my child's academic records and information — including grades, transcripts, report cards, attendance, standardized test scores, and progress reports — from their current or former school(s), and I authorize Bahamas Global Academy to release such records to Henfield Academy. This access is limited to the period during which my child is actively enrolled in and participating in a Henfield Academy program; it begins on the date of enrollment and automatically ends when my child is no longer part of the Academy. The information will be used solely to support academic monitoring, eligibility, and student development, and will be kept confidential, and may be revoked in writing at any time.

Transportation Waiver

I hereby grant permission for Henfield Academy to provide transportation for my child to and from the Academy, athletic training and development sessions, competitions, and other Academy-approved events during the Basketball Training & Development Year. Henfield Academy staff, coaches, or designated representatives may transport student-athletes using Academy vehicles or approved third-party transportation services. I acknowledge that participation in athletic training and related travel involves inherent risks. I voluntarily assume all risks associated with transportation provided by Henfield Academy. In consideration of my child's participation, I release and hold harmless Henfield Academy, its directors, officers, employees, coaches, volunteers, and agents from any and all liability, claims, or demands arising from transportation to and from Academy-related activities, except in cases of gross negligence or willful misconduct. In the event of an emergency during transportation, I authorize Henfield Academy staff to seek and obtain medical treatment for my child. I agree to be responsible for any resulting medical expenses. I confirm that I have read and understood this waiver, and that my consent is given freely and voluntarily.
